NACCHO Annual 2010 Evaluation

NACCHO takes your feedback and suggestions to improve our conference program very seriously. Your responses are integral to our providing you with an even better conference experience in the years to come.  

All conference attendees are asked to complete this online evaluation whether or not you are seeking continuing education credits. However, filling out this evaluation in full is REQUIRED of any individual who registered to receive continuing education units (medical, nursing, health educator, general health professional or Certified in Public Health Continuing Education Credit) either at or prior to the conference.  

Last chance: All evaluations must be completed by Oct. 1, 2010.  

Any attendee who completes this evaluation by the extended deadline will be included in a drawing for one free registration to NACCHO Annual 2011 in Hartford, CT. If you have any questions, please e-mail the NACCHO Annual 2010 Continuing Education Coordinator.
