Resource Center

The Resource Center for Community Health Assessments and Community Health Improvement Plans (CHA/CHIP Resource Center) provides practical, customizable tools and resources to all local health departments (LHDs) in a central and publicly accessible location. The resource center is intended to support LHDs and their partners in completing community health improvement processes, including the conduct of a community health assessment (CHA) and the development of a community health improvement plan (CHIP), for the purpose of improving the health of local communities. The initial version of this resource center is being populated primarily with the following:

  • Existing items that have been deemed high-quality; and
  • Items being developed by LHD CHA/CHIP demonstration sites to complete CHAs and CHIPs for accreditation preparation. 

The CHA/CHIP Resource Center will be updated on an ongoing basis; by December 2012, it will contain example high-quality CHAs and CHIPs and case studies of communities who have completed CHAs and CHIPs for accreditation preparation and more. Check back frequently for new information and updates.

Community Health Improvement Process Models, Frameworks and Guides

Mobilizing for Action Through Planning and Partnerships: Mobilizing for Action through Planning and Partnerships (MAPP) is a community-driven planning process for improving community health. Facilitated by public health leaders, this framework helps communities apply strategic thinking to prioritize public health issues and identify resources to address them. MAPP is not an agency-focused assessment process; rather, it is an interactive process that can improve the efficiency, effectiveness, and ultimately the performance of local public health systems.

NACCHO MAPP Clearinghouse of Resources: The Mobilizing for Action through Planning and Partnerships (MAPP) Clearinghouse was developed specifically for MAPP; however, it also contains useful and relevant resources regardless of the community health improvement process model/framework used.

The Association for Community Health Improvement (ACHI) Community Health Assessment Toolkit: The ACHI Toolkit is a guide for planning, leading and using community health needs assessments to better understand -- and ultimately improve -- the health of communities. It presents a suggested assessment framework from beginning to end in six steps, and provides practical guidance drawn from experienced professionals and a variety of proven tools.

New York State DOH Community Health Assessment Development Page: The NYSDOH has identified 10 steps in the CHA development process by using planning models and frameworks. This website contains descriptions of the 10 steps along with actions and checklists for each.

Community Tool Box, Chapter Three: Assessing Community Needs and Resources: The Community Tool Box, created by the Work Group for Community Health and Development at the University of Kansas, provides information on how to build healthier and more equitable communities. Chapter Three of the tool box ("Assessing Community Needs and Resources") provides practical, step-by-step guidance on how to embark on the community health assessment process.

Guide and Template for Comprehensive Health Improvement Planning, Version 2.1: This guide, created by the Connecticut Department of Public Health, Planning and Workforce Development Section, provides a framework for the development of health improvement plans through approaches that are logical, systematic, and based on historically successful planning initiatives, and is easily adaptable for use by other governmental and community organizations.

CHA & CHIP

County Health Rankings: The County Health Rankings show us that where we live matters to our health. The health of a community depends on many different factors. The website contains a collection of 50 reports – one per state – to help community leaders see that where we live, learn, work, and play influences how healthy we are and how long we live.

Health Equity Index: The Health Equity Index (Index) is a community-based electronic tool that profiles and measures the social determinants (including the social, political, economic, and environmental conditions) that affect health and their correlations with specific health outcomes.

CDC Data Set Directory of Social Determinants of Health at the Local Level: This directory contains an extensive list of existing data sets that can be used to address the social determinants of health. The data sets are organized according to 12 dimensions, or broad categories, of the social environment.

County Health Roadmaps: The County Health Roadmaps show what we can do to create healthier places to live, learn, work, and play. In addition to a tool to build a roadmap for your community, it also contains Roadmaps to Health Action Center which provides tools to help groups working together to create healthier places to live, learn, work and play and theWhat Works for Health database that provides communities with information to help select and implement evidence-informed policies and programs.

Community Commons: Community Commons is an interactive mapping, networking, and learning utility for the broad-based healthy, sustainable, and livable communities’ movement. Registered users have free access to: Over 7000 GIS data layers; contextualized mapping, visualization, analytic, impact, and communication tools and apps; searchable profiles of hundreds of place-based community initiatives; peer learning forums.

Tool: CHIP Template Implementation Plan

This modifiable tool was developed by the Wisconsin CHIPP Infrastructure Improvement Project and is a template for one specific aspect of action planning for impact: a well thought-out implementation plan. Some of the critical parts of this plan include: priority areas, broad goals, specific and measurable objectives, and more.This template will help your community to create a plan that is focused and evidence based and that will help you stay on track.

This modifiable tool was developed by Austin/Travis County Health and Human Services Department (TX) and is a template for their action planning process to ensure it is evidence based. The template is meant to ensure the CHIP is action-oriented and will also them and hospitals achieve specific required related to health improvement.

These modifiable tools were developed by the Austin/Travis County Health and Human Services Department (TX) and are a facilitator's guides for guiding discussion on different aspects of CHIP implementation planning.

These example infographics were created by Dallas County Health and Human Services, TX. Each contains both quantitative and qualitative information for each of the four chosen priority areas.

NACCHO funded 12 local health department demonstration sites to engage in a robust community health improvement process that yielded: a community health assessment (CHA) and a community health improvement plan (CHIP).

Key features of this work included:

  1. Engaging community members and local public health system partners in a meaningful way; 
  2. Addressing the social determinants of health; and 
  3. Using quality improvement (QI) and quality planning techniques. Within the cohort of demonstration sites, NACCHO supported sites in completing several project-specific approaches, or collaborative models.

The CHAs and CHIPs below were developed by the 12 sites as a result of this project and serve as examples of high quality CHAs and CHIPs
