Epidemiology Toolkit
Welcome to Epidemiology Toolkit! NACCHO’s Epidemiology Toolkit offers downloadable tools and resources for local health departments (LHDs). The tools are submitted by LHDs, NACCHO staff and community and national partners to promote information exchange and promotion of best practices and can be adapted for use by other LHDs. Examples of tools in this toolkit include training materials for LHD staff assisting in outbreak investigations, template case report forms, and sample epidemiology profiles.
