The HIV Risk Assessment Questionnaire Part I was developed by the Oregon Health and Science University Partnership Project. This assessment can be administered by staff or can be self-administered. The assessment includes basic questions about HIV risk, such as sexual history, drug use, HIV and STD testing, and closes with a few questions about perception of risky behaviors and intention to change those to be more safe.
