Intake Action Plan

Description:

The Intake Action Plan, developed by the Oregon Health and Science University Partnership Project, is used when clients are enrolled in prevention case management services. The checklist included summarizes the client's background and health history, and concludes with specific goals to be achieved while the client receives services.
