Developed and shared by Burlington County Health Department, these Excel spreadsheets are examples of medication tracking forms, including:
These forms can be easily integrated into local plans and procedures. The tool may be more suitable for smaller health districts because the forms are organized to track bottles rather than boxes. Additionally, this tool does not meet requirements for the tracking of controlled substances.
