CRI Screening form

Description:

This form provides the health department with specific information about the patient receiving antibiotic prescriptions.  The form includes basic information about whether or not the patient may be susceptible to allergy to certain types of antibiotics.  The use of this type of form could prevent allergic reactions in situations in which antibiotics would be used on a large scale.
