Although this tool was created by the Canadian Public Health Association and its partners, it is intended to be adaptable to different contexts. With the goal of bringing a comprehensive approach to regional dialogue and planning by incorporating public health, primary care, and community stakeholders, the tool presents eight Critical Success Factors for strengthening chronic disease prevention and management, with guiding questions for each. It is also supported by case studies of four pilot sites in Canada, as well as a How-to Guide that includes a series of modules outlining the key stages of using the tool.
