Prerequisite: Sussex County Community Health Improvement Plan (2008)

In 2008, the Sussex County Community Health Partnership developed this community health improvement plan (CHIP), one of the three prerequisites for national accreditation. Based on the Mobilizing for Action through Planning and Partnerships (MAPP) process, this 19-page document provides a snapshot of three public health priorities, risk factors associated with the health issues, objectives to strive for to reduce those risk factors, and strategies to help meet those objectives.  This resource is useful for those looking to develop a CHIP in preparation for national accreditation.
