Sample Parent/Guardian Letter for H1N1 Vaccination Clinic Targeting High-risk Children

This one-page letter was used to inform parents or guardians in the Barberton Health District about H1N1 vaccine clinics for high risk children and individuals. Signed by the Health Commissioner, the letter was sent to all the children that were identified as being medically fragile. This effort was coordinated with the local school superintendent, school nurses, and local police department.
