This Diabetes Resource Grid, developed by the Douglas County Health Department (MO), can be used by diabetics, health educators, social service providers, or clinicians. It provides contact information for medical clinics, eye care centers, pharmacies, and dental clinics in the community. And, it indicates whether or not each resource provides the following:

Diabetic and/or prediabetic care,
Diabetic self-care education,
Diabetic education referrals,
Diabetic education follow-up,
Blood sugar testing supplies,
Diabetic medications, and
Prescription assistance.

The Resource Grid can be modified by other LHDs that are looking to provide a similar tool for local providers.
