Effective Performance Improvement (PI) is a data driven process. The use of data enables informed, evidence based decision making. The application of data provides means for initial analysis of how a process is currently performing, assists teams in priority setting, identifying gaps, developing measurable goals and assessing the impact of interventions designed to improve processes and outcomes. This tool, created by students in "Performance Improvement in Public Health" course, shows various ways to create charts and graphs when doing PI work at local health departments.
