Franklin County MAPP for Community Health Improvement

Franklin County MAPP was formed in December 2008 and is supported by the Franklin County Health Department in Frankfort, Kentucky. Using data from the four MAPP assessments Franklin County MAPP developed this Community Health Improvement Plan (CHIP). The CHIP describes our community health improvement process, measurable objectives and improvement strategies, policy changes that are needed for each strategy, individuals and/or agencies that are responsible for implementation and alignment with state and national priorities.
