MAPP Community Health Assessment

Franklin County MAPP was formed in December 2008 and is supported by the Franklin County Health Department in Frankfort, Kentucky.  This MAPP Community Health Assessment (CHA) describes the processes used to gather data and presents both quantitative and qualitative data that was collected during the four MAPP assessments.  The CHA documents all sources and entities that contributed to the health assessment, describes the demographics of our population, specifies population groups with particular health issues, explains contributing causes of community health issues and includes community resources and assets to address health issues.
