In order to ensure that health departments are seeking input from their constituents, measure 9.1.4 A of the PHAB Standards and Measures Version 1.0 requires documentation of "a description of the process used to collect and analyze feedback from two different customer groups."
These files are customer satisfaction surveys developed by the Genesee County Health Department in Michigan. The first file is a PDF of several surveys, the remaining four files are Word Documents so health departments can edit the files to create their own surveys.
