Vulnerable Populations Plan Example

Description:

The Larimer County Vulnerable Populations plan is a comprehensive assessment of the needs of vulnerable county residents.  It includes population estimates for the top vulnerable populations of the county broken down by functional need and the methods of providing information to each of the groups.It also lists which local agencies serve which vulnerable group.  

This plan was submitted as evidence for Goal 1 of a Project Public Health Ready application and can be used as an example by other agencies.
