Health Action: A 2020 Vision for Sonoma County

This action plan developed by Sonoma's Health Action partnership provides an outline of how the county plans to meet their vision of making Sonoma County "a healthy place to live, work, and play... a place where people thrive and achieve thier life potential."  

 The action plan details how the county plans to "create a coordinated, cross-sector, county campaign" to achieve its ten goal areas. It discusses its alignment with state and federal efforts focused on health in all policies.  

For those communities working to develop a community health improvement plan, this provides some example language that might be useful to other communities.
