This 12 page PDF is an example of documentation developed for Goal 2: Measure 4-Training Plan by DeKalb County Board of Health  in 2005 for Round 2 of PPHR. This training plan has an extensive background section which explains the workforce development plan for the DeKalb County BOH.  

The actual training plan is clearly presented in a matrix which comprehensively covers all of the information required for evidence submitted in 2005 for PPHR. This documentation was created based on the Round 2 project criteria. Based on the 2006 criteria, the plan would need to include documentation for rapid-training-curriculum to meet all of the project requirements for Measure 4-Training Plan.
