This 20 page PDF is an example Training Plan submitted to PPHR in 2005 based on the 2004 criteria requirements for Goal 2: Work force Competency. The first page of this example is the template that South Central District Health Department used and a description of the types of information that is contained in the individual learning opportunity matrixes which together make up the training plan.
