ENCompass of Northwest Illinois

Description:

This promising practice outlines how one health department implemented ENCompass of Northwest Illinois, an Internet-enabled community linking system that creates a comprehensive information, referral, case management, and data collection system for service providers and the general population.  The phases of the program are:

  • Service Directory, containing information about all services available to the community
  • Service Referral Process, when clients complete a checklist to identify family needs and is then referred to the appropriate agency
  • Coordinated Service Planning, which enables clients to direct their service providers to collaborate on service plans and goals

Included are details regarding responsiveness and innovation of the program, agency and community roles, costs, and elements in implementation.
