RealBenefits

Description:

This model practice outlines how the Boston Public Health Commission and Community Catalyst have developed Real Benefits, a web-based information service and tool set that was developed to provide human service with the capacity to perform sophisticated benefit eligibility screening, counseling and advocacy for eligible low income families, easily and at low cost.  Included are details regarding responsiveness and innovation of the program, agency and community roles, costs, elements in implementation, and sustainability.
