Making Strategic Decisions about Service Delivery/Partnerships Project

Local health departments (LHDs) must examine their position in providing both population-based and direct healthcare services, as a changing public health environment has led them to transition services and refocus resources to population-based services. LHDs have transferred responsibility to other parts of the healthcare system including community health centers, hospitals, and universities. NACCHO, with support from the Health Resources and Service Administration (HRSA) Bureau of Primary Health Care, developed the Partnerships Project to help LHDs find their niche in the continuum of healthcare service delivery. 

Health Disparities

NACCHO is working with LHDs, federal agencies, and other national organizations to highlight and promote model programs to address health disparities. These programs have been put in a compendium. More »

A number of communities across the country are developing localized models that combine access to a set of health services with care management and outreach to vulnerable populations.
