How to Bring Project Public Health Ready to Your State
 

1. Interested parties must review the Project Public Health Ready (PPHR) Applicant Eligibility Guidelines to determine if they are eligible to participate.

 

2. Eligible agencies contact NACCHO PPHR staff at pphr@naccho.org for more information.

 

3. NACCHO works with eligible agencies and representatives to determine how they may implement PPHR, including identification of a state lead and a NACCHO point of contact. PPHR currently utilizes a state-supported model in which all agencies intending to apply from within the same state must identify a common state lead to serve as a liaison between NACCHO and PPHR applicants. 4. The state lead coordinates with eligible agencies within that state to determine a timeline for all applications. The state lead shares this information with NACCHO and updates projections as needed.

 

5. NACCHO conducts an in-person orientation with the state lead and stakeholders from the eligible agencies within that state. This orientation is conducted when the first group of agencies within that state is ready to being the application process.

 

6. Each of the agencies in the first group of applications must submit an Intent to Apply Form and Part 1 of the application fee 12 months before the intended application submission date. PPHR reviews are conducted annually each fall, with applications due at the end of August.

 

7. NACCHO reviews and approves the Intent to Apply Forms and provides the state lead with the appropriate application requirements and guidance for the designated review cycle.
