Health and Disabilities

Through a cooperative agreement with the National Center on Birth Defects and Developmental Disabilities at the Centers for Disease Control and Prevention (CDC), NACCHO has received funds to support the efforts of local health departments (LHDs) in health promotion for people living with disabilities. The goal is to provide resources to LHDs with practical strategies and recommendations, including tools and materials developed by demonstration sites, technical assistance, and relevant information from partner organizations.

 

The purpose of this project is to:

  • inform and educate local health department staff about disability and health activities;
  • provide financial support to the five current demonstration sites to enhance their efforts;
  • measure progress of demonstration sites; and
  • develop and share model procatices related to health promotion activites for people living with disabilities  
  • Provide technical assistance to local health departments through a peer assistance network.

Technical Assistance

NACCHO is currently offering technical assistance support to local health departments to improve their capacity to develop or enhance health promotion programs for people living with disabilities through coordination of existing health department and community services.

Alameda County, California

Alameda County has a successful Adaptive Diabetes Program with new curriculum for care provider training. Over 140 care providers were trained on the issues of diabetes and disabilities and 45 consumers were trained and learned to make lifestyle changes.

Local Health Department Receiving Technical Assistance:

Jefferson County Public Health Department (WA)

Focus Area(s):

  • Use the adaptive diabetes management and education program for people with developmental disabilities and their care providers.

Jefferson County created a type 2 diabetes program entitled, It Ain’t So Sweet which will be compared with Alameda’s program, the goal being a final program which would have the capacity to build an even greater awareness of diabetes for developmental disabled people and their caregivers.

Central Connecticut Health District, Connecticut

An Advisory Committee was formed and has met several times. The CCHD also conducted a web-based Disability and Health Perceived Barriers survey to assess barriers experienced by individuals with disabilities in regards to health promotion and disease prevention.

Local Health Department Receiving Technical Assistance:

Charles County Department of Health (MD)

Focus Area(s):

  • Bring Healthy Lifestyles to the community, by using the travel stipend to go to a train the trainer program. 

Connecticut will then return to Charles County and provide training to their staff and to interested agencies who provide support services to people with disabilities.  The program will then be implemented.  As a result of the training received, support meetings will be offered which will incorporate Healthy Lifestyles to people living with disabilities in the community and especially to the 18-30 age group with special needs.

Mohawk Area (MA) Public Health Coalition c/o Franklin RegionalCouncil of Governments, Massachusettes

 

Image of a presentationFranklin County, MA made progress in addressing disability emergency preparedness needs by conducting “Community Conversation” meetings; and shifting their focus from a systems level approach to disability and emergency preparedness to building individual and group capacity.

Local Health Department Receiving Technical Assistance:

Lee County Health Department  (IA)

Focus Area(s):

  • Establish a Community Partner Task Force and conduct community partner meetings to develop and coordinate specific strategies for reaching special needs and vulnerable populations in an emergency.
  • Obtain input from the community partners for identifying specific strategies. 

Lee county will review Mohawk Area Public Health Coalition training materials for public education, for human service organizations, and for first responders.  They will adapt Mohawk Area's materials to LCHD’s plan.  Lee County Health Department (LCHD) plans to do an internal assessment of its public health emergency educational messages and materials to assure that they are in a format that is understandable. LCHD plans to address the above areas in more detail with Mohawk Area Public Health Coalition and use technical assistance and resources that can be applied to the above areas.

Allegan County, Michigan 

 

Teacher teaching othersAllegan County trained 18 competent trainers in the Healthy Lifestyles’ curriculum; these trainers have implemented a Healthy Lifestyles’ workshop and ongoing support meetings for older adults and people with disabilities.  The Healthy Lifestyles’ Program is advertised in the ACHD’s newsletter and on their website.

Local Health Department Receiving Technical Assistance:

Madison County - London City Health District (OH)  

Focus Area(s):

  • Train competent trainers in the Healthy Lifestyles curriculum.
  • Implement a Healthy Lifestyles workshop and support meeting for people living with disabilities.

Madison County will support Healthy Lifestyle changes and practices for people living with disabilities by expanding from an individual to a community focus.

Multnomah County, Oregon

Multnomah County created a Steering Committee to direct the project.  A convenience sample survey of Health Department clients with disabilities was conducted; the results were presented to the Steering Committee, and were incorporated into a Health Promotion Summit.

Local Health Department Receiving Technical Assistance:

Akron Health Department  (OH)

Focus Area(s):

  • Conduct a health promotion summit to create an Agenda for Action for health promotion with and for people with disabilities.
  • Identify and engage willing stakeholders.
  • Build consensus among the diversity of members with a variety of agendas.

Akron Health Department is working to gain new perspective on how to assure that their planned summit has a comprehensive agenda, managed, expected outcomes, and planned action steps to achieve an ongoing network of community resources. Akron Health Department is surveying clients/patients with disabilities to identify health promotion needs and interests. In addition, they are working in partnership with leaders and members in the disability community to address the social determinants of health for people with disabilities.
