» Share What Works: NACCHO Model and Promising Practices Database
NACCHO is soliciting Model and Promising Practices used by Local Health Departments (LHDs) to facilitate effective and innovative public health practice in the area of primary care, access to care, LHD-CHC collaborations and health disparities. NACCHO will collect, analyze, and compile new submissions by communities, organizations, LHDs, and other partners specializing in specific subcategories of health initiatives.
