The 2005 Profile instrument was designed as a core questionnaire plus three separate modules. The core questionnaire was sent to every LHD in the U.S. Each module was sent to a random sample (stratified by population served) of approximately 550 LHDs. No LHD received more than one module. A two-page follow-up questionnaire was sent to all LHDs who did not complete the Profile questionnaire. |
