Download the LHD Self-Assessment Tool for Accreditation Preparation based on the Prototype Metrics for the Operational Definition of a functional local health department. |
The local health department (LHD) Self-Assessment Tool for Accreditation Preparation is based on the Operational Definition Prototype Metrics. This self-assessment tool allows LHDs to measure themselves against the Operational Definition standards and subsequently identify areas of strength and areas for improvement.
LHDs that are planning to become accredited through the national program would be well-served to assess the degree to which they have the capacity to fulfill the functions outlined in the Operational Definition, and engage in quality improvement activities as needed to enhance their capacity to this end.
