Accreditation and Quality Improvement Demonstration Sites Project

The 2009 Model Practice Award Program recently recognized 23 initiatives that demonstrate exemplary and replicable qualities in response to identified public health needs. Among these model practice award recipients are six LHDs that participated in NACCHO's Accreditation Preparation & QI Demonstration Sites Project and submitted applications describing their unique QI efforts:

  • City of Milford Health Department (CT)
  • Coconino County Health Department (AZ)
  • Douglas County Health Department (MO)
  • Lawrence-Douglas County Health Department (KS)
  • Lee County Health Department (IA)
  • Osceola County Health Department (FL)
An additional nine demonstration sites were recognized as promising practices. Congratulations to all of this year's awardees!

 

With funding from the Centers for Disease Control and Prevention and the Robert Wood Johnson Foundation, NACCHO created the Accreditation Preparation and Quality Improvement (QI) Demonstration Sites Project to give local health departments (LHDs) from across the country the opportunity to start preparing for accreditation. From November 2007 through May 2008, 10 LHDs participated in the first round of the project, where the sites conducted self-assessments using the LHD Self-Assessment Tool for Accreditation Preparation to identify areas for improvement and then implemented QI projects to address priority areas identified by the self-assessment.

From April 2008 through November 2008, the project was expanded for a second round and included 56 participating LHDs. Of the 56 sites, 31 LHDs worked individually to implement QI projects much like the round one sites, while the remaining 25 LHDs worked in five regional collaboratives. These collaboratives conducted self-assessments to decide on priority improvement areas to address and then engaged in planning processes to develop formal mechanisms of collaboration for future work.
