Project Overview: The Kane County Health Department (KCHD) serves a population of 550,000 in an urban-suburban area of Illinois about 35 miles west of Chicago. KCHD identified staff expertise and training on collecting, managing integration of, and displaying data as an area for improvement. They then developed a community-tailored and user-friendly template to display collected data that relates to the community action plan goal of reducing maternal-child health disparities in Kane County.
