Implementing Healthy People

With fewer resources and staff, many local health departments (LHDs) face significant challenges in providing essential services that ensure the health and safety of their communities. The HP 2020 approach offers an evidence-based, easy-to-use tool designed to aid LHDs and other organizations in community health assessment work.

The examples below provide guidance to help LHDs and other organizations use HP 2020 to conduct community health assessment and improvement planning processes.

Community Health Assessments

»	Delaware General Health District
	The Partnership for a Healthy Delaware County (OH) published a report on Key Findings and Priorities of MAPP Research in 2008. Delaware General Health District compared local data with HP 2010 national targets to indicate where additional public health resources needed to be focused. More »

»	Santa Barbara County Public Health Department
	Santa Barbara County Public Health Department (CA) developed the 2011 Community Health Status Report to present information about the leading causes of death and disability among Santa Barbara County residents. Santa Barbara County compared local data with HP 2020 goals to inform decision making regarding program and policy development and resource allocation. More »

»	St. Louis County Department of Health
	St. Louis County Department of Health (MO) and the University of New England published the St. Louis County Community Health Needs Assessment report in 2011. The report highlights HP 2020 national targets as a means for comparing local and national data. More »

Community Health Improvement Plans

»	St. Clair County Health Department
	The St. Clair County Health Department and County Health Care Commission (IL) developed a comprehensive community health assessment of needs and a five-year community health plan in fulfillment of the Illinois Department of Public Health IPLAN Certification Requirements for LHDs. The St. Clair County Health Department used the MAPP model and HP 2020 topic areas and objectives. More »

	The Will County (IL) Community Needs Assessment and Strategic Plan utilizes HP 2020 objectives to identify and address priority community health problems. More »

Progress Trackers

»	Hidalgo County Health Department
	Professional Research Consultants created a Health Forecast for Hidalgo County (TX), an interactive tool designed to make community health needs assessment data widely available to the communities they reflect. The Health Forecast compares local data from Hidalgo County to state data, national data, and HP 2020 goals. More »

»	San Francisco County
	San Francisco County (CA) created a HP 2020 Progress Tracker to monitor San Francisco County's advancements towards HP 2020 national targets. HP 2020 progress trackers allow communities to assess their health status and build an agenda for community health improvement. More »
