Demonstration Site Case Studies

In 2001, NACCHO selected nine communities to serve as demonstration sites for the newly developed MAPP tool. NACCHO was able to follow six of the nine original communities through the six phases of MAPP. The following documents describe each community's experience and the process they went through using the MAPP tool. The case studies also provide links to resources used and lessons learned. For more information about any of the communities, contact mapp@naccho.org.
