Guide to the Profile Study Data, 1990-2005 This document is a useful guide to the data available in the Profile study from 1990 to 2005. More » 2008 Profile Instrument The 2008 Profile instrument was designed as a core questionnaire plus three separate modules. The core questionnaire was sent to every LHD in the United States. Each module was sent to a random sample (stratified by population served). No LHD received more than one module. 2005 Profile Instruments and Codebooks The 2005 Profile instrument was designed as a core questionnaire plus three separate modules. The core questionnaire was sent to every LHD in the United States. Each module was sent to a random sample (stratified by population served) of approximately 550 LHDs. No LHD received more than one module. A two-page follow-up questionnaire was sent to all LHDs that did not complete the Profile questionnaire. Instruments: Codebooks: 1996-1997 Profile Instrument and Codebook The 1997 Profile had a response rate of 88 percent, with 2,492 local health departments out of 2,832 responding. 1992-1993 Profile Instrument The 1993 Profile had a response rate of 72 percent, with 2,079 local health departments out of 2,888 responding. 1989-1990 Profile Instrument The 1990 Profile had a response rate of 77 percent, with 2,269 local health departments out of 2,932 responding.
