This new tool will provide guidance in using the Healthy People 2030 objectives and targets at the local level in a community health improvement process. It is designed to assist local health departments in moving their community health improvement work upstream by selecting objectives related to the Social Determinants of Health, and identifies tools from Healthy People 2030 to be used in setting targets. The toolkit also provides a framework for facilitating strategic alignment between partners to achieve shared goals. Access the toolkit here.
