Request for Applications: Family Healthy Weight Program Action Institute

Purpose

NACCHO’s Maternal, Child, and Adolescent Health team, in partnership with Veritas Management Group, is launching an application for a new initiative, the Family Healthy Weight Program (FHWP) Action Institute. This action institute will bring together organizations currently implementing FHWPs to document implementation challenges, develop reimbursement strategies, and create policy solutions that can be elevated to state and federal levels. This year-long, non-funded initiative will result in a repository of evidence-based reimbursement strategies that can inform future policy changes to support the sustainability of FHWPs.

Applications launch on Thursday, May 1, 2025, and close on Monday, June 15, 2025. All necessary information regarding the action institute and the application process may be found in the Request for Application (RFA). Notifications will be sent to applicants in July 2025.

NACCHO and the Veritas Management Group hosted an information session on Thursday, May 8, 2025, to provide an overview of this non-funded opportunity and the application process. The recording for this information session will soon be available for applicants to review on NACCHO’s website.
