With support from the National Center for State, Tribal, Local and Territorial Public Health Infrastructure and Workforce at the Centers for Disease Control and Prevention (CDC PHIC), NACCHO is offering a funding opportunity for a consultant to develop and facilitate a virtual Local Health Department Change Management Community of Practice (LHD Change Management CoP) for a subset of LHD COVID-19 Health Disparities Grant recipients who are members of the Health Equity Sustainability Cohort. The selected consultant will be awarded up to $34,700 to develop and facilitate a CoP that will support health equity change leaders within LHDs in successfully implementing lasting organization-wide strategic and operational changes that support the sustainability of health equity strategies and activities. This CoP will cover the steps of a change management process, create an understanding and awareness of how human beings react to change, build the critical skills needed to successfully move through the process of change, as well as provide opportunities for members to share experiences, tools, and resources related to change management in the context of local public health.
