Education is the first step to understanding the social determinants of health and making on impact on health outcomes. Our team developed a presentation to take to professionals and key stakeholders in our community to demonstrate and provide tools to identify and subsequently address the social determinants of health.
Live Well Madera County’s Community Health Improvement Plan goals will focus exclusively on upstream preventative intervention. In addition to CHIP goals to address the two strategic priorities, diabetes and obesity and child abuse and neglect, the over 25 Live Well partners agreed the CHIP will incorporate Health in All Policies approaches in their agency practices.
Visitors to Yosemite National Park might have seen her ⎯ a young woman working as a housekeeper at one of the hotels catering to the thousands of tourists who stream through Mariposa County every year.
Every weekday around noon, local residents, workers, high school students, and visitors to Mariposa County head to the parking areas near the Mariposa History Museum in search of their favorite street food. The local food truck culture attracts people of all ages and walks of life, drawn by the aromas of two taco trucks and a hamburger joint on wheels.

The Maricopa County Department of Public Health (MCDPH), implemented the “Improve ABCS” and "Increase Physical Activity" strategies during this Million Hearts® demonstration period, to empower residents of public housing to improve their...
Navajo County is in the process of capturing an equitable assessment of our community’s health needs. To assist us in the development of a cohesive, county wide Community Health Improvement Plan (CHIP), we have partnered with many agencies, organizations and community members, within and outside of the health care industry. Our goal is to accurately reflect the health needs and gaps in the...
The opioid epidemic has spread rapidly and uncontrollably through our country and our community. Research consistently links Adverse Childhood Experiences and Adverse Community Environments, the Pair of Aces, with opioid use and abuse. Ending this threat requires a coordinated, community-wide effort between non-traditional partners to address the immediate crisis as well as the root causes.
How can a local health department respond to an onslaught of vector control (mosquito and rat) complaints? Easy, develop a special revenue fund to wholly fund an integrated pest management program. This self-sustaining funding mechanism allows for a proactive program aimed at minimizing the vector control disease burden.
