The new Getting Further Faster Community of Practice (GFF CoP) aims to empower community coalitions and multi-sector partnerships, including state and local health departments.
To help address the social determinants of health and advance health equity, NACCHO partnered with the HHS Office of Disease Prevention and Health Promotion to develop this guide on using Healthy People 2030 objectives and targets at the local level in community health improvement.
Building upon the results of Years 1 and 2 of the CDC's Improving Social Determinants of Health (SDOH) – Getting Further Faster (GFF) initiative, this funding opportunity will build evidence for how public health, healthcare and human services systems can implement, evaluate, and sustain programs to promote cross-sector linkages that address the social determinants of health (SDOH).
