Actions to Implement a CHIP

LHDs take a variety of actions to implement or sustain their community health improvement plans (CHIPs), including developing or strengthening relationships with community partners, participating in a coalition to address one or more priorities, and establishing or reaffirming priorities for LHDs. The chart below represents the percentage of LHDs involved in each type of activity within the past three years.
