To help address the social determinants of health and advance health equity, NACCHO partnered with the HHS Office of Disease Prevention and Health Promotion to develop this guide on using Healthy People 2030 objectives and targets at the local level in community health improvement. This guide is designed to provide a structure to align MAPP and Healthy People 2030 to assist local health departments and their communities in creating prevention strategies and using collective action for change across the whole community.
