As part of a quality improvement (QI) project, the Kane County Health Department (KCHD) identified staff expertise and training on collecting, managing integration of, and displaying data as an area for improvement. They then developed a community-tailored and user-friendly template to display collected data that relates to the community action plan goal of reducing maternal-child health disparities in Kane County.
