In 2008, the City of Milwaukee Health Department was awarded a grant by NACCHO through the Creating Local Community Information Networks: A Local Public Health Informatics Demonstration Project, through support from the Centers for Disease Control and Prevention. The toolkit contains information about the project including evaluation, challenges, successes, lessons learned and future plans. The implementation of this project led to real-time reporting of patient data from all hospitals in the County. The module of this project highlights the automated generation of schedules report for hospitals infection control. Additionally, it enables the hospital infection control professional to retrieve additional patient information from Wisconsin Health Information Exchange (WHIE). This toolkit will be useful for local health departments preparing to implement electronic medical records or integrate a health information exchange.
