Missoula City-County Health Department has developed a Community Health Assessment (CHA), Community Health Improvement Plan (CHIP), and Health Department Strategic Plan that explicitly include people with disabilities. This health department was a recipient of technical assistance from the NACCHO Health and Disability Project Team and these plans can be used as a model for local health departments working to increase disability inclusion and engagement. The 2014 CHA includes data on the demographics of people with disabilities in the jurisdiction, a health profile of disabilities, health and social services for people with disabilities, and transportation for people with disabilities. The CHA also specifically recognizes that people with disabilities experience preventable health inequities. The 2015-2018 Strategic Plan includes specific goals and objectives for serving people with disabilities, and the 2015-2018 CHIP includes people with disabilities in strategies, action plans, and activities.
