This one-page PDF document provides a sample of an H1N1 influenza surveillance report created by the Boston Public Health Commission's Communicable Disease Control Division. The division developed a surveillance system to capture important streams of data that informed Commission officials of how the epidemic was impacting city residents. The report shows data from spring and autumn 2009, including reported cases by age group, emergency department visits for influenza-like illness, race/ethnicity distribution of confirmed cases, geographic distribution, and syndromic and school surveillance. In addition to improving situational awareness for public health officials, these surveillance reports helped quickly identify disparities in the burden of illness. Additionally, the Commission's use and interpretation of surveillance data during the 2009 H1N1 response improved decision-making, strengthened communication with its partners, and helped identify and address response gaps. This tool was submitted as part of an H1N1 Story from the Field. The story can be found at this link: http://www.naccho.org/topics/H1N1/h1n1story.cfm?story_id=324
