This is an archived program. Resources are still available although grant funding has ended. Responding to changes in public health and healthcare delivery, local health departments (LHDs) are examining their roles in providing both population-based and direct healthcare services. LHDs have begun to transition services and refocus resources to population-based services, transferring responsibility to other parts of the healthcare system such as community health centers, hospitals, and universities. To address the challenges posed by a changing public health environment, NACCHO developed the Partnerships Project, which will help LHDs find their niche in the continuum of healthcare service delivery.
