Getting Further Faster Community of Practice (GFF CoP)

We are excited to announce a new Community of Practice designed to provide resources to address social conditions to improve community health and prevent chronic disease. The new Getting Further Faster Community of Practice (GFF CoP) aims to empower community coalitions and multi-sector partnerships, including state and local health departments. The primary goal is to build capacity to amplify, accelerate, and sustain their efforts to advance health equity by addressing social determinants of health (SDOH) within communities.

Connect with public health peers across the country, as well as with health care and community organizations. This platform encourages the sharing and learning of knowledge and experiences building coalitions. The GFF CoP goes beyond networking, offering evidence-based resources and tools, and coaching and mentoring to support local coalitions.

Register here for our next peer-learning event, Capacity Assessment: Understanding Where We Are to Get to Where We Want to Go, on Thursday, February 15, 2:00 to 3:30 p.m. ET. We will discuss how multisector community coalitions can use capacity assessments to identify organizational strengths and opportunities to address social determinants of health (SDOH) and plan for future work. This free event will feature two coalitions: the Food as Medicine Collaborative and Access Health Stark County. The coalitions will share their experiences conducting capacity assessments (1) to gauge their partnerships’ and local ecosystems’ capacity to implement new state policy and (2) to build their data systems’ capacity to measure progress, respectively. The GFF CoP is supported by the Centers for Disease Control and Prevention’s (CDC’s) National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP), the Association of State and Territorial Health Officials (ASTHO), the National Association of County and City Health Officials (NACCHO), and community health coalitions across the United States.
