Improving Social Determinants of Health – Getting Further Faster Project Releases Three RFPs

NACCHO is pleased to announce the release of three Request for Proposals to support partners’ program evaluation, communications, partnership development, health equity, and sustainability efforts related to employing social determinants of health approaches to improve chronic disease.

Evaluation, CQI, and Data Use Capacity

The National Association of County and City Health Officials (NACCHO) and the Association of State and Territorial Health Officials (ASTHO) have selected 42 community multi-sector partnerships/coalitions across the country to receive over $2 million as a part of the Improving Social Determinants of Health – Getting Further Faster evaluation program. NACCHO has released a Request for Proposal to fund a training and technical assistance up to $350,000 to support these community multi-sector partnerships/coalitions participating in evaluation, continuous quality improvement, and data use capacity. Multi-Sector Partnership Development and Sustainability
