RFA: Building Multi-Sector Partnerships to Advance Health Equity in Cardiovascular Disease Prevention Efforts

With support from the CDC, NACCHO is offering a funding opportunity to local health departments to enhance coordination across community leadership to strengthen cardiovascular disease prevention efforts by addressing social determinants of health (SDOH) and advancing health equity. NACCHO will make up to three (3) awards available with awards up to $30,000 each. For full details about the funding opportunity, please review the Request for Applications here.
