Social Determinants of Health in Rural Communities Toolkit

Health and well-being is influenced by many factors outside of just medical care received. The Social Determinants of Health in Rural Communities Toolkit assembles evidence-based and promising programs and resources to assist organizations in implementing programs to address social determinants of health in rural communities.

This resource was released by The Rural Health Information Hub (RHIhub) and created in collaboration with the NORC Walsh Center for Rural Health Analysis.
