Since 2003, the National Association of County and City Health Official's (NACCHO) Model Practice Awards program has honored initiatives—including programs, resources, and tools—that demonstrate how local health departments and their community partners can effectively collaborate to address local public health concerns.

NACCHO is pleased to honor this year's recipients of Model Practice and Promising Practice awards, which will now be apart of the online, searchable Model Practice database. Selected from a group of 93 applications, 24 public health programs were selected as "model," and 47 public health programs were selected as "promising".
