The COVID-19 pandemic exposed significant challenges in vaccine access and confidence, particularly among young children, adolescents, and underserved communities facing cultural and historical barriers to healthcare. Local Health Departments (LHDs) lead efforts to address these challenges, yet success hinges on building partnerships with trusted community institutions.

With funding from the Centers for Disease Control and Prevention (CDC), NACCHO funded 25 entities to support LHDs in establishing sustainable, community-centered partnerships with Early Care and Education (ECE) providers, faith-based organizations, and community messengers supporting adolescents and their families.

The project was extended from September 2024 to June 2025, five of the 25 sites did not extend. Those sites success stories are highlighted  HERE. You can also access the success stories by clicking on the below graphics.
